GRADUATION APPLICATION PACKET INSTRUCTIONS
The following instructions should help the graduation candidate prepare ALL the paperwork needed for graduation. Please read the following directions very carefully.
When you complete the procedures listed below, you will be ready to submit the items as your Graduation Application Packet.

PROCEDURE FOR COMPLETING THE GRADUATION APPLICATION PACKET
1. Graduation Application (Electronically through EKU Direct) ***DO NOT FORGET STEPS 2, 3 and 4 if applicable***

 2. Alumni Association Information Sheet (Print, complete and submit to Dean's office)

3. Survey Completion Page (Please submit to Dean's office with Alumni paperwork)
   *Disable the pop-up blocker on the browser if you are having trouble accessing any of the links below. Also, if a page appears "There is a problem with this website's security certificate", this is normal. It is just a safety measure your computer uses to ensure that it is communicating with the correct website. Please click "Continue to this website (not recommended)".
  >ALL students the "Survey of Undergraduate Degree Candidates" will become available through your EKUDirect during your last semester. Complete this survey and submit.
  >Print the “Thank you for completing this survey” page or Completion Verification Form for each survey you complete.
